Temporary Licenses


How to apply for a temporary Speech/Language or Audiology license:

  • Submit a completed and notarized application.
  • Submit the fee per the Fee Schedule. The temporary license may be renewed only with Board approval.
  • Request that the official college transcript of your master's program be forwarded to the Board office.
  • Submit documents verifying completion of at least 375 hour of supervised practicum. (Letter from practicum supervisor is acceptable).
  • Request national test scores be forwarded to the Board office. Clinical fellowship year candidates have one year under the temporary license to submit test scores.

How to apply for a temporary Hearing Aid Dispenser license:

  • Submit a notarized application.
  • Submit the fee per the Fee Schedule. The temporary license may be renewed only with Board approval.
  • Submit verification of high school diploma or its equivalent.
  • Submit Training Plan. The Training Plan must be signed by the Hearing Aid Dispenser trainee and their supervisor. The Training Plan form is available on this web site or can be obtained by contacting the Board office.
