Applying for a Reciprocal License

**AS OF JANUARY 5, 2008 ALL APPLICANTS FOR ORIGINAL, RECIPROCAL OR TEMPORARY LICENSE WILL BE REQUIRED TO OBTAIN A CRIMINAL HISTORY BACKGROUND CHECK**

Before applying for an original license you must:

  • Possess a baccalaureate or graduate degree in health and human services, hospital administration, nursing, or business administration or hold a current Delaware RN license.
    • Or posses an associate’s degree in any field or hold a current Delaware RN license.
  • Complete an AIT program and meet certain experience requirements.

Submit:

  1. Completed, signed and notarized application to the Board's office, along with the appropriate pro-rated fee. Check or money order should be made payable to the "State of Delaware".
  2. Certification of NAB examination scores sent directly to the Board office from NAB. (See www.nabweb.org).
  3. Official Criminal History Background Check from the State Bureau of Identification (SBI) or a statement from the SBI that the State Central Repository contains no such information relating to you the applicant.**
  4. Official report of your entire federal criminal history record pursuant to the Federal Bureau of Investigation (FBI) of Title II of Public Law 92-544.**
  5. Official college transcripts to be sent directly to the Board office from the educational institution.
  6. Verification of licensure in good standing from the state(s) where currently licensed or has ever held a license as a Nursing Home Administrator sent directly to the Board Office from each state’s licensing authority.
  7. In order to determine if your state meets the substantially equivalency requirement please submit to the board with your application, a copy of the other jurisdiction's licensing laws and regulations for evaluation. If your state is not substantially equivalent, please follow the directions under License Requirements – New License.
  8. Responses to Section 4: Occupational Background and Section 5: Administrative Background of the application form.
When all the required documents have been received by the Board, the application will be placed on the agenda for the next scheduled meeting. The completed application and all requirements must be received at least two (3) business days prior to the next Board meeting.
