Applying for a New Dental Hygienist License
The Delaware Practical Examination in Dental Hygiene is offered twice a year at the beginning of January and mid-May. The application deadlines are December 1 and April 1. The exam is limited to 36 candidates on each date. See Fee Schedule for exam fee. For information about the exam, click Dental Hygiene Exam.
To apply to sit for the exams, submit:
- Completed, signed and notarized application
- Pro-rated processing fee by check or money order made payable to the "State of Delaware."
- Copy of current cardiopulmonary resuscitation (CPR) certification card
- Proof of high school graduation or GED
- Certified transcript showing graduation from a Board-approved dental hygiene program, sent directly from the school to the Board office
- If dental hygiene program is not completed by the application deadline, submit a letter from school officials stating
- that you are in good academic standing and
- the expected date of your graduation
When you are notified of a passing score on the examinations, submit the following:
When your application is complete, the Board will review it and you will be notified of the Board’s decision.
Applying for Dental Hygienist License by Reciprocity
You may apply for licensure by reciprocity if you:
- hold a current license in another state, and
- have worked 3 of the last 5 years.
If you do not meet these conditions, you must sit for the Delaware Practical Examination in Dental Hygiene.
Submit the following:
- Completed, signed and notarized application
- Pro-rated processing fee by check or money order made payable to the "State of Delaware"
- Certified transcript showing graduation from a Board-approved dental hygiene program, sent directly from the school to the Board office
- National Dental Hygiene Board Examination score report showing at least 75% competency level
- Copy of current cardiopulmonary resuscitation (CPR) certification card
- Letter(s) of reference from past or current employers attesting to your good moral character and reputation
- Verification letters from state(s) in which you are now, or have ever been, licensed
- Resume/curriculum vitae showing work history for the last five-year period
- If you have a criminal record, copy of your criminal history record from the State Bureau of Identification
- W2 forms from 3 of the last 5 years to prove work in a dental setting
When the Board has approved your application, you must successfully pass the Delaware Jurisprudence Written Examination. |
