HOW TO REQUEST A LICENSE ROSTER

The following information is considered public information and can be disclosed:

1. Licensee Name
2. License Number
3. License Type
4. License Status
5. Issue Date
6. Expiration Date
7. City and State
8. Public Disciplines

Public disciplines are not available on rosters at this time. To obtain a licensee's disciplinary history, please submit a Written Verification Request, or click Verify License Online.

Mailing and email addresses are not considered public information. We cannot release them.

Rosters are in Excel spreadsheet format. We request that you supply an email address to which we can send the roster as an attachment.

Please allow 10 business days for processing your request.

1. Complete the information on the form below. All license types under the jurisdiction of the profession(s) you select will be included on the roster.
2. Enclose the appropriate fee to be paid by check or money order made payable to the "State of Delaware." Cash and credit card payments are not accepted.

Professions other than Nursing: $30 per profession

Nursing:

1. Registered Nurses Only: $66
2. Licensed Practical Nurses Only: $30
3. Advanced Practice Nurses Only: $18
